Change to the way you receive your medications on discharge from hospital. If you are an inpatient in hospital you should be given up to 28 days supply of any medications that you need to take at the time of discharge.
Funding has been diverted from primary care to hospitals to cover this change. Your GP will no longer be able to provide a prescription for any medication for the first 28 days after hospital discharge. However you will need to make an appointment with your Doctor to discuss the need for ongoing medication after the 28 day period.
If you did not receive your medications on leaving hospital please ring the ward as soon as possible. If you have any comments or complaints about this change in policy, please contact the Prescribing Unit of the Eastern Health and Social Services Board on 9032 1313.
